Initial Visit


Before each of your pet's first appointment, we ask that you fill out our new client paperwork. We maintain separate files for each patient, therefore we ask that you fill one out for each pet. After you have filled out the form please print it and bring it to your first appointment along with any vaccine or previous medical history you have for your pet. We ask that you arrive 10 minutes prior to your appointment to allow us time to process your paperwork and get you into your appointment on time. Travel Visit


Before traveling with your pet please familiarize yourself with the laws both in the United States and abroad. We have provided some general guidelines about traveling with your pet.

International travel with your pet can be a tricky and rather difficult process. To help facilitate this procedure we have provided a set of guidelines. The initial exam is a minimum of an hour. The first half will be spent with the staff going through the extensive paperwork, while the second half will be the medical exam with your veterinarian. All follow up appointments must be scheduled with the same veterinarian as required by many countries.

Boarding Visit


Please bring the completed consent form to the clinic when you drop off your pet for boarding.
